Medicaid Home- and Community-Based Waiver Services are available to a limited number of patients as an alternative to ICF or IDD placement. The waiver allows individuals to receive services, such as care coordination, non-medical transportation, supported living services, environmental modifications, adaptive equipment, consultation services, supplemental support services, and crisis intervention.

These services are only made available by a 245D licensed community provider like Care Focus.

Companion Service

When you choose Care Focus for companion services, you receive more than a caregiver. From ongoing senior care to short-term care during illness and/or recovery, we act like an extension of your family, helping bring you peace of mind and improved quality of life at home or in a healthcare setting.

Covered companion services include:

  • Practicing coping skills in different settings
  • Accompanying a patient to an event to reduce social isolation
  • Playing board games to aid in the enhancement of fine motor skills
  • Provide cues or verbal instructions to help a person complete a task
  • Assist or supervise the patient with tasks such as housekeeping, meal prep, shopping, and laundry

Night Supervision

Night Supervision Services involve overnight monitoring and assistance provided by a staff member in the client’s home. the covered services include;

  • Carrying out the client’s positive support programming and transition program
  • Reinforcing skill development supports, such as independent living skills
  • Assisting with activities that are essential to everyday life.

These services are provided for up to 12 hours in a 24-hour period.

24-Hour Emergency Assistance

24-Hour Emergency Assistance includes immediate on-call counseling and problem-solving. This also covers personal or health emergencies. This service is given to clients living in their homes.

To be eligible for 24-Hour Emergency Assistance, the client must meet one of the following:

  1. The client must live in his/her own home, doesn’t already receive 24-hour supervision, and would otherwise require extensive supervision on a routine basis.
  2. The client lives with a primary caregiver who would require extensive support in the event of an emergency.

There are equipment and non-equipment services that are provided under this type of assistance. Non-equipment services include immediate response to the need for assistance and on-call problem solving and counseling. Equipment services include Personal Emergency Response System (PERS) installation and testing, as well as PERS monitoring.

Personal Support

Personal Support Services are provided to a client in their home or assisted living community. The goals are to help the client achieve more by increasing his/her independence. If living in a community, inclusion goals are a priority because they are important for the patient.

The exact goals are set based on the outcomes specified in the client’s support plan. Teaching and training may not be required to reach the outcomes in the plan. This is also the least costly form of assistance that still gives a client what they need. Covered services include:

  • Assistance with daily living activities
  • Community service access
  • Developing meaningful relationships with members of the community
  • Nurturing existing relationships with members of the community
  • Participating in community activities the client chooses

This is a one-on-one service that is provided outside of the home so the client can interact with people with and without disabilities. This isn’t a habilitation service, so it may not be the only waiver service that a person can take advantage of.
